Passport Applications

PASSPORT FEES EFFECTIVE FEBRUARY 1, 2008

* The $25 Execution Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form.

IMPORTANT NEW PASSPORT INFORMATION

1. New Passport Application Forms
   Updated versions of the following forms are now available at Acceptance Facilities:
   • DS-11, Application for a U.S. Passport
   • DS-82, Application for a U.S. Passport by Mail
   • DS-4085, Application for Additional Visa Pages & Miscellaneous Passport Services
   • DS-5504, Application for a U.S. Passport (Name Change, Data Correction, Limited Passport Replacement)
   • DS-3053, Statement of Consent or Special Circumstances; Issuance of a Passport to a Minor Under Age 16
2. New Passport Card
   Customers will now have the option to apply for a Passport Book, a Passport Card or Both on the new DS-11, DS-82 and DS-5504 forms.  The Passport Card may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda and the Caribbean.  The Passport Card will provide a less expensive, smaller and more convenient alternative to those who travel to these destinations.
3. New Fees
   Please be advised that the fee chart above reflects fee changes effective February 1, 2008:
   • Execution Fee
   • Passport Book/Card Fee (DS-11, Application for a U.S. Passport)
   • Passport Book/Card Fee (DS-82, Application for a U.S. Passport by Mail)
4. Two-Parent Consent Age Change
   P New Requirement: Two parent consent required for minors under the age of 16
                   Children under the age of 16 who apply for the passport card must use the DS-11 Form and pay the $25 Execution Fee even
                    if they already have a fully valid Passport Book.
